In Virginia be sure to register with the VDH and I would think it's the same elsewhere. Stay registered, my friend.
People are also having good success with small private pharmacies and grocery store pharmacies. It seems that many of the ones with supplies that aren't that busy as more flexible with the rules. Look for a small pharmacy tucked in a medical building.
Look into the walk-in situation at various clinics, large and small.
For all of this, be aware that the announced restrictions about the group being vaccinated , e.g. 65+, are not firmly or uniformly enforced, so don't let that discourage your search.
Around here at least, the communications for appointments are inconsistent and sometimes unreliable. Emails go to spam/junk. Phone calls won't show the source so start answering everything.
Often you get a call/email for an appointment that is the next day or so. Be ready to jump on it.
Typically when you get your first shot, they schedule the day, but not the time, for the second. Scheduling the second can be last minute so block off the day. Also, some people feel crappy the day or next day after the second, so plan accoordingly.
I don't know the rural v urban dynamics.
